REQUIRED QUALIFICATIONS:
Knowledge of:
- Knowledge of and experience working with people living with sickle cell disease.
- Understanding sickle cell dissease and the unique needs this patient population has.
- Knowledge of ethnically and racially diverse populations .
- Brief assessment and familiarity with grief and loss.
- Knowledge of healthcare system operations.
- Identify potential physical, psychological, social and spiritual impacts of sickle cell disease.
- Basic word processing and computer functions.
Ability to:
- Exemplify Be The Match Standards.
- Maintain professional awareness, knowledge and understanding of specific issues that sickle cell patients live with.
- Create and maintain relationships with patient-focused sickle cell advocacy groups.
- Communicate effectively with patients, caregivers, families and the public to build trusting relationships across a broad range of socioeconomic and cultural backgrounds.
- Employ active listening, encourage active communication and solutions-oriented in interactions with patients, families and members of the health care team; to optimize patient outcomes .
- Communicate effectively with navigator colleagues, health professionals and health related agencies to promote patient navigation services and leverage community resources to assist patients.
- Demonstrate empathy, integrity, honesty and compassion in difficult conversations.
- Apply insight and understanding about emotions and human responses to emotions to create and maintain positive interpersonal interactions.
- Demonstrate sensitivity and responsiveness to a diverse patient population, including but not limited to diversity in gender, age, culture, race, religion, abilities and sexual orientation.
- Effectively communicate via phone and in writing.
- Achieve Oncology Patient Navigator-Certified Generalist (OPN-CG) or Oncology Nurse Navigator – Certified Generalist (ONN-CG) Certification within first 2 years of employment.
Education and/or Experience:
- Bachelor’s degree in social work, human services, nursing or health care related field. May consider applicants with an Associate’s degree in a health care related field with at least 3 years of relevant work experience.
- Three years of experience working directly with patients to provide assessment, education and navigation. Or upon evaluation, a master’s degree in a healthcare related field and one year of minimum experience.
PREFERRED QUALIFICATIONS: (Additional qualifications that may make a person even more effective in the role, but are not required for consideration)
- Medical knowledge in the area of oncology and/or stem cell transplantation.
- Second language a plus.
